What is the DS-64?
DS-64 reports a valid U.S. passport that was lost or stolen so it can be cancelled. Officially, it is the Statement Regarding a Valid Lost or Stolen U.S. Passport Book and/or Card . Reporting it protects you from identity theft and keeps anyone else from using it.
Use it only for a passport that was still valid when it was lost or stolen, an expired passport does not need this form. Once you report it, the passport is cancelled for good and cannot be used again, even if you find it later. If you are replacing it, you can submit DS-64 with your DS-11 application.
