What is the SSA-454-BK?
SSA-454-BK is the report Social Security mails to people already receiving SSDI or SSI when it's time to check whether they're still disabled. Officially, it is the Continuing Disability Review Report (SSA-454-BK) . It isn't an application, and there's nowhere on it to sign — Social Security gets your medical records directly and mostly uses this report to know where to find them.
The report covers your medical conditions, providers, medicines, and daily activities, plus work and education once you're 14 or older. Sections on support services, other medical contacts, and literacy only apply if you're 18 or older.
